For ongoing home oxygen at prescribed flow rates, an oxygen concentrator is usually the right choice — it makes oxygen from room air indefinitely and costs little to run. A cylinder stores a fixed amount of oxygen and is best as backup for power cuts and for transport. Most homes on oxygen therapy should have both: the concentrator as the workhorse, a cylinder as insurance.

How each actually works

A concentrator is an electrical machine that pulls in room air, strips the nitrogen, and delivers concentrated oxygen through a cannula — for as long as it has power, without ever 'running out'. Home units commonly deliver up to 5 litres per minute, with 10-litre machines for higher prescriptions. Running cost is essentially electricity plus periodic filter care.

A cylinder is stored, compressed oxygen — no electricity needed, silent, and delivering full concentration at whatever flow the regulator sets. Its limitation is arithmetic: a cylinder holds a fixed volume, and at continuous use even large cylinders empty in hours to a day or two, after which refilling is a trip, a cost and — during shortages — a scramble every family who lived through 2021 remembers.

Add a cylinder as power-cut insurance and for transport; in cities with unreliable supply, this is not optional, and an inverter that can run the concentrator is worth pricing.

Occasional or emergency-only use is the cylinder's honest niche — but be careful with the idea of 'keeping oxygen just in case': oxygen is a prescribed medicine, and self-started oxygen can delay the hospital visit that was actually needed. If someone's saturation is dropping, the priority is medical attention, with oxygen as the bridge a doctor advises — not a substitute for the call.

Rent or buy — the honest arithmetic

Rent when the need is weeks to a few months (post-illness recovery, post-surgical support, trial phases) — rental spreads cost, includes service, and ends when the need ends. Buying starts winning when a machine will run many months to years, as in COPD's long arc — several months' rent roughly equals a decent machine's price. Buy through the arithmetic, not the salesman: multiply the monthly rental by your doctor's honest estimate of duration, compare with purchase price plus servicing, and decide. Either way, insist on a pulse oximeter and a written target range — the machine without monitoring is comfort theatre, and monitoring is what makes home oxygen safe.
